I wondered the same thing, so did a bunch of searching. I came up with the movie Bell, Book, and Candle (1958), as well as a number of references back to a witch hunter in 1647, and one of the witch's failiars had that name. In the movie, the familiar is, of course, a siamese cat.

Kim Novak is one of my all time favorite actresses and she plays the coolest witch of all time. Her kitty is her familiar and she uses it to cast her spells.
